While I have seen mention of these rare items, I don't recall any specifics of who might have made them in the past. Any words of wisdom from the TMers?

Krazy Horse had one made for me.

I suggest you experiment with stand-offs to work out the right spacing. I initially tried a 2" spacing but found it made the indicator stalk too far away. A 1" spacing gives me comfortable room for my knees when I lift my foot off the clutch, leaves the indicator stalk in reach, and makes the car easier to get into than 2", and I am 6' 5". There's a thread here on how I fitted the spacers.

(I feel safer about the twisting issue raised there with the 1" stand offs too)
