Slip a folded up cloth or towel beneath the spare wheel so that it doesn't slip down and the holes should line up with a little gentle persuasion. The cloth or towel can be removed once the T bar has "caught" on the threads of the mounting. A flexible rather than rigid pipe from the grease gun body to the nipple makes things a lot easier. Normally it is just a case of turning the road wheel to expose the nipple - much easier if the car is on a jack and axle stands with the key in the ignition to stop the steering lock operating.
