Phil is correct.
I have in the past had the well documented crankshaft pulley failure, where the bolt fixing this pulley tries to leave the engine through the front cover.
The recent new event this thread relates to regards a camshaft pulley which ended up spinning on the end of the camshaft it is supposed to drive, as the key fixing the pulley to camshaft had eaten away at the pulley leaving nothing to drive.
