I cannot get in or out of my 3W without removing the steering wheel. When I think the wheel is fully engaged on the shaft, I give it a good tug... twice. Once, giving it the "safety tug", it came off. I had not fully engaged it. Once properly engaged, it's not going to come off by itself.

Sad indeed, and the steering wheel not on the vehicle is a big clue. I would wager he was not using the locknut to insure the wheel stays put.

Seems strange to be blaming the steering wheel: in the forces in a crash I could imagine the sleeve around the column maybe disengaging the release mechanism. But losing the wheel during a crash event would surely be a good thing? In any case v sad to read.

Sad news indeed, especially if the incident was attributable to incorrectly fitted Q/R wheel and could so easily have been avoided.
Sometime before my 2016 car was built one of the three steering wheel attachments was fitted by the factory with a longer bolt to limit movement of the spring loaded removal collar. Many, including myself replaced this with shorter to allow quick removal. There are two sets of splines the wheel needs to fit over when re-attaching. It is possible to fit it located on just one although the excess movement is fairly obvious.

Truly awful. Irrespective of the underlying cause of the accident, what is without question is that the steering wheel wasn't present in the vehicle after the crash. Either, it came away before the crash and the driver had no control of the vehicle, or in the impact it came off. Either way, this is not a sign of good engineering and/or design. The Coroner has recognised this. "Mr Ellery (the Coroner) said he will write to Morgan to recommended that the company reviews the safety of the removable steering wheel." This isn't a bland statement as the Chief Coroners "Guidance No 5 Reports to Prevent Future Deaths" ( https://www.judiciary.uk/wp-content...o-5-reports-to-prevent-future-deaths.pdf) makes amply clear. Essentially, MMC has to look at this urgently. My understanding of the law is that they have 56 days to respond.

I learned to go out and get into the MTW without taking off the steering wheel. On the other hand, I control severely if the steering wheel is well attached before starting .....

There are a couple of threads re: this RTC in the 3W section of this forum. Bill was a (sorely missed) fellow tour guide. Unfortunately, it would appear we will never know the full reason for this crash. Appeals for witnesses seem to have failed.
