Obie, Surely the release adapter is a permanent fit on the steering column.
It looks like the adapter has come off attached to the steering wheel.

No nothing has come off John, when fitted correctly all is secure.
Its just that with the 20 ish splines on my QR the wheel will go on in the wrong position, not enough to engage the retainer but enough for the wheel to feel like it is attached securely but could come off with the obvious dangers.
This is only a problem if someone didn't know how to use the QR and fail to check that the retainer is engaged.
It should either go on if lined up correctly or not at all so something is wrong with the design or the splines on my particular setup.

It looks like the adapter has come off attached to the steering wheel. John, the release adapter is affixed to the steering wheel and not permanently attached to the steering column.

Just to prove I’m not going mad here’s a video. Very. This horrid accident clearly asks questions about the steering wheel fastening.... I had a removable wheel on my Lotus. It had a very simple push on to the see/feel that 2 opposite red 'studs' had popped out, indicating the wheel was correctly and securely fastened... but even then, always gave it a big pull and wiggle to check... I hope MMC quickly get on top of this. I had not realised that a removable wheel was standard/or an option.... Can the car be 'lived in' with a standard fixed wheel?

Agree Pete, very worrying.
At least the video shows that it Is in fact possible to put the wheel on in the wrong place and for it to feel firmly attached.

My quick release is a keyed, splined fitting that only fits on one way. When fully engaged onto the splined steering column all the way to the recess in the steering columns splined shaft it makes a destination click. It also is evident that it is fully engaged by the one long bolt on the quick release mechanism will almost be flush with the quick release flange and the ball detents will engage the recess in the splined steering column so the nyloc nut will still be able to keep the flange from being pulled to the rear and stop the wheel from being removed.
It is a pretty foolproof system if used as designed. To NOT use that safety nut properly a driver is asking for potential problems.

I think I'm going to set up a "pre-flight procedure" because this is a sobbering video. I can totally see myself carried away and not fastening the steering properly.
