Looks as though the survey's set up to require an answer in every box. Just put a dot or similar in any box you don't want to complete. Then it works!

Allows you to fill in the questionnaire, but on submission returns a red error "This question requires and answer" - all are answered and there is no question highlighted as still requiring an answer...
Belay that - each line in the address box needs an entry...

I got the red error message on the first attempt because no 'Company' name entered. Typed in 'none' and it accepted ok.

Fill the COMPANY box in with anything. I put in N/A and it worked but I am sure anything will do. Lets hope its a good mag.

Did that. Still an error, then I added the area of Cheltenham where I reside, so no blank lines in the address and it accepted that.
