The belt can ride on the flanges on the rear hub/sprocket and make terrible noises also. The belt should ride here.

My belt has always run 2-3 mm off the side of the rear sprocket without any adverse effects. I have the Supermax rear sprocket and no flange. In reverse it sits square in the centre as your picture above but as soon as you drive forward it is back to 2-3mm off the side.
I stopped worrying about it years ago.
