Keith at BHM told me that because some fuses are marginal, they can develop hairline cracks, so test ok, but when heated up will fail, only to cool and start working. So probably a hairline crack that would pass a test current but not full voltage.
That makes sense.
The moral of the story is that if the logic says fuse "X" has blown, just change it.
