Can you take a video of the car on start-up?
The car idle for months won’t help either. No movement/warming up of linkages etc could be contributing.Do the simple checks on plugs/leads/choke/carb/fresh fuel as a first step. If no nasty noises get the car up to temp. Short run to loosen it up. If OK then Italian tune up? Do it in stages for your peace of mind.
It is easy to bypass the small simple checks and worry about major problems instead. Eliminate the simple and easy ones first.
