No nothing has come off John, when fitted correctly all is secure.
Its just that with the 20 ish splines on my QR the wheel will go on in the wrong position, not enough to engage the retainer but enough for the wheel to feel like it is attached securely but could come off with the obvious dangers.
This is only a problem if someone didn't know how to use the QR and fail to check that the retainer is engaged.
It should either go on if lined up correctly or not at all so something is wrong with the design or the splines on my particular setup.
