Years of underfunding, some very odd contracts being awarded have resulted in disastrous IT projects throughout the public services, in particular the NHS, resulting in poor IT and massive cost over runs.
Exactly the same in the private sector. The big consultancies push a quote that is very low whilst spreading rumours and disinformation about the alternatives. Then wen they get the contract they try to fulfil it with the cheapest possible resources and use every excuse to pile cost in for additional requirements and changes.
From what I hear about the Excel cockup it was a failure of basic data design, they were trying to put data tuples in columns rather than rows, something an A level student would immediately see was stupid.
