Besides marking the original position of both sides of the axle and counting the number of turns to fully loosen the drive belt what else should I be aware of?

So if you don't remove the wheel from the hub during a tire change, is the wheel balance then done on the car?

Every tyre change I've done I've balanced it myself on a balancer that I used when I raced bikes. Apart from the sheer size of the wheel and tyre, the general arrangement is much more like a bike wheel than a car wheel.

Are the cones that go into the hubs ball or roller bearinged?

Neither...they are just a sliding fit on the axle which runs on a pair of bearings at each support post

Right. But there is not a normal center for cones in the M3W wheel.

There is, the cones slip inside the inner race of the wheel bearings which are mounted in the hub. Most of these balancers use simple o-rings on the axle outside the cones to stop them slipping out. Even with the weight of the M3W wheel, hub, sprocket and drum it works more than adequately.

I might have a go at making one. A shaft, a couple of tight tolerance roller bearings in a stand and a couple of centering cones. Neat winter project.

BTW, where did you get that one if I get lazy?
