Definitely seen better days! What you need will be a new pair of rod end bearings, also known as Rose Joints or Heim Joints depending on which side of the pond you are looking for them. Size should be M14x1.5 female thread but worth checking that MMC didn't fit something else that day... There is a bewildering array of different quality and prices for seemingly the same size bearings but generally something that is recommended for motorsport use and PTFE lined is a good bet. Don't lubricate them if they have a PTFE or similar lining. Only grease the type that come with a grease fitting; but do grease that type frequently. Fit them at best guess for alignment then get the alignment checked and adjusted. The M14 Allen screws that go down through the "Comfort Kit" cones can get a bit tight in the cones and take a bit of effort to get them out. If so it's worth de-burring the insides of the cones a bit before re-assembly.
