There are several reasons why a cycle path may not be used. Often the cycle paths accumulate detritus as cars have a cleaning effect on tarmac and push the stones and rubbish to the side - where the cycle path is.

The reason I don't use cycle paths locally is the council have placed the entry at an angle, thus producing a tipping force on three wheelers (I have to ride trikes - no balance from the ears).

There are some well thought out cycle paths but in my area, I can only think of one. The others are more about meeting targets than actually providing a decent alternative.
