A similar thing happened with a Supermax sprocket at G&G in the Lakes a few weeks ago (photo below). The poly overlay disbonded and the grub screws sheared off or worked lose, not sure which happened first. The owner stopped and investigated this at the first occurrence and found the problem before any damage was done to the poly overlay.

The car was recovered and the rear wheel removed. The overlay was then bonded back on with epoxy adhesive and new grub screws fitted. All refitted and the M3W returned to action the following day. Other Supermax sprockets have had the grub screws work lose, they need to be secured with Loctite. Bunny mentioned this was an issue with Supermax sprockets in April 2019.
