For those of us who do not have remote greasing, is it preferable to have the wheels on the ground when greasing or is it okay to jack up the car so the wheels are off the ground when greasing? Obviously access is easier with wheels off the ground.

Any greasing is better than none. However, with the wheels off the ground, more of the king pin is inside the hub, so arguably gets a more direct splash of grease.
I have fitted extenders to my top nipples which makes it really easy to apply a grease gun by simply bending over to access under the top of the wheel arch rather than crawling underneath, as both nipples feed the same grease gallery.
I grease every 500 miles and I only tend to use the wheel arch method when touring, for a quick squirt if I am going to be well over the 500 mark, to save crawling underneath in good clothes.
