I suspect you could well be right, if the nozzle isn't properly engaged onto the nipple then you won't be able to pump grease in.
But having said that, I wouldn't worry. The top nipple is pretty much redundant. I only ever grease the lower nipples and have never had a problem. Some people like to pump heavy oil into the top, but I've never really seen the need.
